I hereby agree to participate in the activities associated with the 2025 International Gathering, December 3-8, 2025, organized and facilitated by Shadhiliyya Sufi Center, Inc., (the "Organizer"). In consideration for being allowed to participate, I acknowledge and agree to the following terms for myself, my family, and my dependents:

I understand and accept the following terms and conditions:(Required)

Assumption of Risk: I UNDERSTAND AND ACKNOWLEDGE THAT THE ACTIVITIES I AM ABOUT TO ENGAGE IN (CLASSES, GROUP WORSHIP, SING-A-LONGS) MAY INVOLVE INHERENT RISKS AND HAZARDS. I AM VOLUNTARILY PARTICIPATING WITH FULL KNOWLEDGE OF THESE RISKS, INCLUDING BUT NOT LIMITED TO PHYSICAL INJURY, ILLNESS, PROPERTY DAMAGE, AND LOSS, INCLUDING OF FUTURE EARNING CAPACITY. I HEREBY EXPRESSLY AND SPECIFICALLY ASSUME ALL ASSOCIATED RISKS, INCLUDING ANY AND ALL RISK OF INJURY, HARM, OR LOSS THAT I MAY INCUR AS A RESULT OF MY PARTICIPATION IN THE ACTIVITIES. THE TERMS HEREOF SHALL SERVE AS AN ASSUMPTION OF RISK FOR MYSELF, MY HEIRS, ESTATE, EXECUTOR, ADMINISTRATOR, ASSIGNEES AND FOR ALL MEMBERS OF MY FAMILY.

Release and Waiver: I HEREBY FULLY AND FOREVER RELEASE, DISCHARGE, AND EXPRESSLY WAIVE ANY AND ALL KNOWN OR UNKNOWN CLAIMS, LIABILITIES, CAUSES OF ACTION, AND DEMANDS OF WHATEVER KIND OR NATURE, EITHER IN LAW OR EQUITY (“CLAIM” OR “CLAIMS”) OR AGAINST SHADHILIYYA SUFI CENTER, INC., ITS OFFICERS, DIRECTORS, SUBCONTRACTORS, AGENTS, AND REPRESENTATIVES ("RELEASEES") THAT MAY ARISE FROM MY PARTICIPATION IN THE ACTIVITIES OR ARISE OUT OF OR ARE RELATED TO ANY LOSS, DAMAGE, EMOTIONAL DISTRESS, OR INJURY, INCLUDING DEATH, THAT MAY BE SUSTAINED BY ME, MY DEPENDENTS, OR MY PROPERTY WHILE PARTICIPATING IN THE ACTIVITIES, WHETHER ARISING OUT OF THE ORDINARY NEGLIGENCE OF THE ORGANIZER OR ANY RELEASEES OR OTHERWISE. I COVENANT NOT TO MAKE OR BRING ANY SUCH CLAIM AGAINST THE ORGANIZER OR ANY RELEASEE, AND FULLY AND FOREVER RELEASE AND DISCHARGE THE ORGANIZER AND OTHER RELEASEES FROM LIABILITY UNDER SUCH CLAIMS INCLUDING NEGLIGENCE OF THE ORGANIZER OR RELEASEES. THE TERMS HEREOF SHALL SERVE AS A RELEASE FOR MYSELF, MY HEIRS, ESTATE, EXECUTOR, ADMINISTRATOR, ASSIGNEES AND FOR ALL MEMBERS OF MY FAMILY.

INDEMNIFICATION: I AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASEES FROM ANY CLAIMS, ACTIONS, DAMAGES, JUDGMENTS, COSTS, OR EXPENSES, INCLUDING ATTORNEY'S FEES, THE COSTS OF ENFORCING ANY RIGHT TO INDEMNIFICATION UNDER THIS RELEASE, AND THE COST OF PURSUING ANY INSURANCE PROVIDERS, ARISING FROM MY OR MY FAMILY’S PARTICIPATION IN THE ACTIVITIES, MY OR MY FAMILY’S BREACH OF THIS AGREEMENT, OR ANY DIRECT OR THIRD PARTY CLAIM AGAINST OR INDEMNIFICATION OBLIGATION OF RELEASEES BY OR OF FORT LAUDERDALE NORTH MARRIOT AND THEIR RESPECTIVE OFFICERS, DIRECTORS, PARTNERS, AGENTS, MEMBERS AND EMPLOYEES ARISING OUT OF OR CAUSED BY ME OR MY FAMILY’S NEGLIGENCE, BEING ON THE PREMISES, OR PARTICIPATING IN ACTIVITIES.

Medical Treatment: In the event of any injury or illness during my participation, I hereby give consent and authority to the Organizer to seek medical treatment on my or my family’s behalf and at my expense. I understand and agree that I am solely responsible for all costs related to such medical treatment, medical transportation, evacuation, and all other related costs. I hereby release, forever discharge, and hold harmless the Organizer from any claim whatsoever in connection with such treatment or other medical services. I understand and agree that the Organizer assumes no responsibility for any injury or damage, which might arise out of or in connection with such authorized emergency medical treatment. The terms hereof shall serve as a release for myself, my heirs, estate, executor, administrator, assignees and for all members of my family.

Personal Property: I am solely responsible for the safety and security of my and my family’s personal belongings during the activities and release the Organizer from any liability for loss, theft, or damage to my and my family’s personal property.

Media Release: I grant the Organizer the right to photograph and video and audio record my family and Me during the activities. I also grant the Organizer the right to livestream my family and Me via Zoom during the activities. I understand that these images and recordings may be used for future promotional purposes and I waive any rights or claims to compensation for their use. The terms hereof shall serve as a release for myself, my heirs, estate, executor, administrator, assignees and for all members of my family.

Governing Law and Venue: This agreement shall be governed by and construed in accordance with the laws of Wyoming, without regard to its conflict of laws principles. Any claim or cause of action arising under this Release may be brought only in the federal and state courts located in Laramie County, Wyoming and I hereby consent to the exclusive jurisdiction of such courts.

Miscellaneous: I hereby agree that this Release represents the full understanding between the Organizer and Me and supersedes all other prior agreements, understandings, representations, and warranties, both written and oral, between us, with respect to the subject matter hereof. If any term or provision of this Release shall be held to be invalid by any court of competent jurisdiction, that term or provision shall be deemed modified so as to be valid and enforceable to the full extent permitted. The invalidity of any such term or provision shall not otherwise affect the validity or enforceability of the remaining terms and provisions. This Release is binding on and inures to the benefit of the Organizer and Me and our respective heirs, executors, administrators, legal representatives, successors, and permitted assigns. Section headings are for convenience of reference only and shall not define, modify, expand, or limit any of the terms of this Release.
