Zakah Recommendation Form

Dear community leader, please fill out this formonce for each member of your community who needs and is qualified to receive zakah and zakat al-fitr this year. For example, if you are recommending 3 different people, complete this form 3 times.

Zakat al-Fitr will be distributed on Day 20 of Ramadan and zakah will be distributed about one week after Ramadan ends, insha’allah.
